Team Members and spouses enrolled in a Bloomin’ Brands medical plan can earn Health Rewards and save on medical premium costs for the following plan year. As part of the Bloomin’ Brands Health Rewards program, you must complete a physical exam each plan year.

With a Catapult Health VirtualCheckup Home Kit, you can complete your annual biometric screening from the comfort of your home to earn next year’s medical premium discount.

Health Activities: More Ways to Earn Rewards
Complete Health Activities throughout the year to earn your medical premium discount for the following plan year.
- To qualify, you must earn at least 100% during the current plan year. Additional activities may allow you to exceed 100%, but earning at least 100% is required to receive the discount.
- If your spouse is enrolled in your medical coverage and also completes the program requirements, you’ll receive an additional medical premium discount.
What Are Health Activities?
When it comes to your health, small steps can lead to meaningful, long-term results.
- Complete preventive health screenings, including annual checkups, dental exams, vision exams, and other recommended wellness screenings, to stay on top of your health.
- Connect a compatible fitness tracker or health app to automatically track activities such as steps, exercise, and workouts. Regular activity can help you build healthy habits.

Complete Your Health Assessment & Health Health Activities
- Activate your Health Rewards Program by first completing your Health Survey. This required first step unlocks the remaining Health Rewards activities and opportunities to earn progress toward your annual goal.
- Complete your annual preventive checkup to stay on top of your health.
- Complete eligible preventive screenings, such as dental and vision exams, to support your overall well-being.
- Connect a compatible fitness tracker or health app and participate in daily or weekly activities. Tracked activity can help you build healthy habits while earning credit toward your Health Rewards Program.
- Monitor your progress throughout the year and complete Health Rewards activities to earn at least 100% and qualify for the medical premium discount for the following plan year.
Note: The Health Survey must be completed first before additional Health Rewards activities become available.

**These percentages are accumulated based on the activity frequency. Daily activities accumulate each day they are completed, while weekly activities accumulate each week they are completed, up to 365 days or 52 weeks during the plan year.
Additional Information
All wellness activities must be completed between Jan. 1 and Sept. 30, 2027 to receive the premium discount as of Jan. 1 2028.
Activities completed between Oct. 1 and Dec. 31, 2027 will still be accepted. Any premium reductions earned after Sept. 30 will take effect as soon as administratively possible after BBI receives notice of completion.
Premium discounts will not be retroactive to Jan 1. 2028.
Individual results are not shared with Bloomin’ Brands.

Earn Rewards for Healthy Living
Annual Physical or Biometric Screening
As part of the Bloomin’ Brands Health Rewards program, you must complete either a biometric screening or a physical exam each plan year.
With a Catapult Health VirtualCheckup Home Kit, you can complete your annual biometric screening from the comfort of your home to earn next year’s medical premium discount. Catapult Health has replaced Quest biometric screenings starting in 2026.
Health Actions: More Ways to Earn Rewards
You can complete Health Actions to earn points and get rewarded with a medical premium discount for the following plan year. In order to receive the medical premium discount, you must earn at least 200 points during the current plan year. If your spouse is enrolled in your medical coverage and completes the same requirements, you’ll receive an additional medical premium discount.

Interesting challenges offer activities to tackle and perhaps share with coworkers, friends or family members. You can set goals and keep track of your progress easily on your computer or mobile device.

- Log in to your My Health Toolkit account. Click Create an account if you don’t already have one set up.
- Select Wellness and then Personal Health Assessment to complete the questionnaire.
